Certified
Copies of Vital Records:
To obtain a certified copy of a
marriage, death, or birth record please submit a letter of
request to us. This letter should contain the person's name
and date of record. It is also necessary for you to send
us a photocopy of your driver's license or birth certificate
for identification purposes. Send a letter of request to
our office at 850 Fairfax, P.O. Box 308, Carlyle, IL
62231. Please make the check or money order

Marriage
License:
To obtain a marriage license the
couple must come to the office to answer license
questions. A photo identification is required. Both
parties must sign the license. You must get your license
at least one day before your marriage and used within 60
days. The license is obtained in the county that you are
getting married. The fee for a marriage license is
$15.00. No blood test required.

Assumed Name
Certificate
To apply for an assumed name
certificate the business owner must fill out a form.
This form can be obtained from our office in person or by
mail. The fee for filing for this certificate is $5.00.
After it is filed in our office it must be published for
three consecutive weeks in the newspaper of your choice in
Clinton county. Publication cost will be your
responsibility. After we receive the certificate of
publication from the paper, we in turn will send you a Assumed
Name Certificate.

County Liquor
License
To apply for a county liquor license
the business owner must fill out an application form.
This form is obtained from our office in person or by
mail. After the application is completed, the sheriff s
office does a background check on the applicant. If
there are no problems a liquor license is issued. The
cost of the license varies with the closing time
requested. Yearly liquor licenses expire on June 30th
annually and must be reapplied for each year.
Beer only
single event license is also available.
